Required Forms

Complete all forms in their entirety for participation in the competitive sport program which includes Middle School Teams (grade 6th to 8th) and High School Teams (grade 9th to 12th).

All student-athletes must register on AthleticClearance.com , starting May 15th, 2026.

Downloadable Forms / Information Sheets

• EL2 - Preparticipation Physical Evaluation Form

This form must be downloaded and completed by an approved medical professional. Once completed, it must be uploaded to the AthleticClearance.com portal.

• EL1 - ECG Screening Form

This form is required for all rising/incoming 9th graders and new student-athletes entering in grades 10-12.  Once the form is completed by the appropriate medical professional, please upload it to the AthleticClearance.com portal.
